It does not replace clinical judgment or orders. Every result is a suggestion based on the values entered. Follow provider orders and assess the patient. If anything here differs from the official, currently approved protocol, the official protocol takes precedence.

Verify before you act. Insulin is a high-alert medication. Check every rate against the printed protocol and complete independent double-checks required by facility policy before changing a pump. Results are only as accurate as the values entered.

Protocol version. Built from the VASD DKA/HHS Algorithm and insulin titration tables updated June 2026. If the protocol has been revised since then, this calculator may be out of date.

Approval status: pending review by pharmacy and the protocol owner. This tool has not been reviewed or cleared by the FDA.

Built-in assumptions. Insulin rates assume a 1 unit/mL concentration and are rounded to 0.1 unit/hr. Effective osmolality uses (2 × Na) + glucose ÷ 18, which differs from the formula printed on the protocol. Corrected sodium is sorted as low (<135), normal (135–145) or high (>145), cutoffs the protocol does not define.
